A heavy brow or a slightly dropped lid is a cosmetic problem. It is visible, it is distressing to the patient, and it resolves. The lower third is a different category of risk, because the muscles you are treating are doing work the patient depends on continuously.
Over-treat the DAO and the smile becomes asymmetric in a way the patient notices in every photograph. Get the mentalis wrong and the lower lip loses competence. Diffuse into the orbicularis oris after a lip flip and speech and drinking are affected for weeks. Dose the masseter carelessly and chewing changes, or the muscle bulges paradoxically at the wrong point in the treatment cycle.
None of that is a reason to avoid the lower face. It is the reason the lower face is taught after 50 upper-face treatments rather than instead of them, in a cohort of four where every injection is watched, and with dosing taught as a ladder you climb across treatment cycles rather than a number you memorize.
The recurring theme through the day is conservatism as technique rather than timidity. You can always add at two weeks. You cannot take away.
Every lower-face zone is taught with a starting dose deliberately below the eventual target and a two-week review built into the plan. This costs an appointment and prevents the outcome nobody can reverse.
Assessment starts with what the muscle does for the patient — speech, oral competence, mastication, expression — before what it does to their appearance. That order is what keeps lower-face work safe.
